Improve Tracking of Surgical Implants

Opportunity

Less than 5 percent of electronic documentation for surgical events using mesh included the minimum descriptors of product code and lot number for procedures performed at North Shore Hospital and Elective Surgery Centre (ESC) during 1 July 2017 – 30 June 2018. This information is  captured retrospectively from the paper theatre notes by the Clinical Coding Team to adhere to the Ministry of Health directive regarding mesh implant tracking.

Aim

To develop a process to correctly record descriptors of product code and lot number for 100% of surgical patient events containing implant products by December 2018.

Based on the cause and effect analysis and process map, the team considered a range of solutions with varying degrees of required effort and likely impact:

Conclusions

The resolution needs to be expandable to other surgical implant types, and meet the needs of various stakeholders (e.g., Finance, Procurement, Clinical Coding, Quality and Risk).
